Updated quantile functions

Improved quantile functions, and smoothed estimation near the mode.

Stable release for Sam's paper

This release includes

• 3 families for tail distributions (gaussian , student t, generalized normal)
• 5 families for observation error (gaussian, lognormal, binomial, poisson, negative binomial)
• random effects as optional in mean / variance
• fixed effects formulas to create design matrices for mean + variance

Update: include fixed effects

Previously, only fixed random were included -- but this version includes options of

trend + random effects
random effects alone
fixed effects alone
All of the above can be included in the mean (mu) or standard deviation (sigma)
Also, the instability in the inits is fixed.
