The script produces the following Markdown:

How unlikely is that? <...> by lightning[^1][^2] <...> (although probability can be [tricky][3]).

[^1]: Cooper, Mary Ann, MD. ["Disability, Not Death is the Main Problem with Lightning Injury"][1]

[^2]: [][2][http://www.nws.noaa.gov/om/hazstats/light08.pdf][2]

(The [][2] empty link is present in the source, it is not a script problem.)

In my opinion the numbers of the links should follow the order of appearance in the text:
