It's too restrictive to require CDATA for documents

The spec currently requires CDATA tags only for wrapping documents and results. Both CDATA and encoded text should be acceptable since XML parsing libraries usually can handle either transparently and using encoded text prevents having to deal with nested CDATA tags.
