Ensure item number column includes all relevant items associated with cat 3 errors

Per discussion below in #3122, only the first item number is populated in the item number column for category 3 errors:

Because this change is too big to implement in that PR, this ticket will deliver the change.

Please confirm, should we expect only one item number on cat 3?

That is correct. It should pull the correct item number based on which field presents the error (almost always the result_field but i believe there's a case where condition_field gets reported)

We show multiple field names, but only one item number, so i could see an enhancement following. But I couldn't squeeze that into this PR without a bunch more changes
