I think this is due to a race condition, as it only happens when CPU load is high. Reproducible on my Win10 machine.

Computer was suspended. When resumed, CPU load was high. Switched to web browser (Firefox) and Passifox it immediately triggered Keepass (via PassiFox Firefox Add-On / KeePassHttp KeePass Plug-In). Since I had just resumed the keepass database was locked and I was prompted to unlock. I clicked Cancel to the keepass password prompt. Somehow passifox kept triggering keepasshttp which repeatedly failed, then I got this:
