I tried quickly the form. Looking for a company was a bit sketchy. I didn't have time to check how personaldata.io/wikipedia is handling the list of dpo's. CNIL is publishing its own one. Might be useful to get at look at it: https://www.data.gouv.fr/en/datasets/organismes-ayant-designe-un-e-delegue-e-a-la-protection-des-donnees-dpd-dpo/
