If the report contains errors and the user fixes them, on submission the address/dorm is repeated the number of times the user clicked submit before fixing the errors.

Issues seems to be coming from demographics.jsp where bothe the Dorm and Address field are pointing to the same DAO object. Need to update the database with a dedicated field for holding Dorm info.
