Use <figcaption> for all images. Right now the caption is placed inside alt= but it is not easily visible.

This is best addressed generally by WYSIWYG text editing. It should integrate with images, formulas and relations. Will file an issue shortly

Captions are a bit different; general WYSIWYG do not handle figures. The text inside IEV definitions actually conform to the StandardDocument model. Unless you are talking about Metanorma WYSIWYG...
