Review question list editor interface

NB. Other issues should be resolved first.
The list editor is hard to use on small screens. It would be nice if we could improve this by allowing the user to change the size of the question list and the question bank, or possibly hide the question bank. It is worth looking at the table design too.
This is not a technical task, but rather a design task which requires some discussion.
