mappings/chapterdata.csv contains two Structure ADT_A01 rows #46

https://github.com/HL7/v2-to-fhir/blob/master/mappings/chapterdata.csv#L803 seems to be incorrect. Line https://github.com/HL7/v2-to-fhir/blob/master/mappings/chapterdata.csv#L816 is the correct ID for the "Cancel Discharge / End Visit" Name and What.

803:Structure,ADT/ACK,Cancel discharge/end visit,ADT_A01,,,,,
816:Structure,ADT/ACK,Admit/Visit Notification,ADT_A01,,,,,

Also can the white space be removed after ID in the header?

Cristy: Can you clarify what you mean to be sure? Is it the What, Where, When row? Can you check in the actual IG (build - HL7.FHIR.UV.V2MAPPINGS\V2 to FHIR - FHIR v4.0.1<https://build.fhir.org/ig/HL7/v2-to-fhir/branches/master/index.html>) what may be inaccurate as the .csv you are referencing in gitHub would be there for the transition from Google Spreadsheets to the IG.
