Thold plugin execute a script each time device state changes? v2

This is a reference to issue/request #82

What was done for this works fine. The initial idea though was to have command executed on device up/down (thold dead/up device notifcations)
You can't really do that, you can only run a script to a defined
cacti/settings.php?tab=alerts&

Can you please also make the scripts available here?

Basically thresholds do 2 major things:

1. Checks if some threshold is breached. Say my firewall is reaching 100 degrees, alert is issued, command is being ran. That's great and all is fine.
2. Checks if firewall is down or up. No command execution can be done in this case, only mail notification. Or at least I haven't found it. See screenshot above.
